SAMD 266«Медицинское заключение о допуске к участию в физкультурных и спортивных мероприятиях, ГТО»

SAMD 266 is a medical report on the patient's admission to physical education and sports events, educational and training events, sports competitions and compliance with the standards of the TRP complex.

The document allows you to record the decision on the patient's admission to various types of physical activity, as well as indicate the limitations of physical activity, if any. After signing, the document is sent to the RAMD.

Creating a document

1. Schedule the patient for a routine appointment.
   To form a document, the patient's profile must specify:

• Last name, first name.
• Paul.
• Date of birth.
• SNILS.
• The subject of the Russian Federation in which the patient lives or is registered.

2. Create a new protocol with the type «Медицинское заключение о допуске к участию в физкультурных и спортивных мероприятиях»and fill in the date of birth of the patient.

Characteristics of the medical care provided

If necessary, click the button «Добавить»and specify the characteristics of the medical care provided.

After that, the fields will appear:

• The form of medical care.
• Type of medical care.
• Conditions of medical care.
  

The added characteristics can be deleted using the trash icon.

Medical report data

Fill in the details of the medical report.

Required fields:

• The registration number of the report.
• The registration number of the person (physical education student, athlete).

If necessary, you can also specify:

• Date of issue of the medical report.
• The name of the event.
• A type of sport.
• Sports discipline.
• The stage of sports training.

Specify the admission decision separately for each type of event:

• Admission to educational and training events.
• Admission to participate in sports competitions.
• Admission to participate in physical education events.
• Admission to compliance with the standards of tests (tests) of the TRP complex.
  

Also indicate if the patient has any limitations, including physical activity.

In the block«Ограничения, в том числе физических нагрузок», select one of the options:

• «Есть ограничения».
• «Ограничений нет».
  
  If selected«Есть ограничения», specify:
• «Срок ограничения».
• «Описание ограничений».

The field «Описание ограничений»is required for the Unified State Register of Legal Entities.

Fill in the required fields:

• «Медицинское заключение действительно до».
• «Заверяет документ».

If necessary«Страховые полисы», select the patient's insurance policy in the field.

Signing

After filling out the document, you can sign or print it out.

If the required fields for the Unified State Register of Legal Entities are not filled in, the Medlock will display a message«Заполните обязательные поля», and the button «Подписать»will be unavailable.

Printed form

The printed form of the medical report displays:

• Information about the medical organization.
• OGRN of a medical organization or individual entrepreneur.
• The registration number of the medical report.
• Last name, first name and patronymic of the patient.
• Date of birth of the patient.
• The registered number of the person, if available.
• The name of the event.
• A type of sport.
• Sports discipline.
• The stage of sports training is subject to availability.
• The decision on admission to training events.
• The decision on admission to sports competitions.
• The decision on admission to physical education events.
• The decision on admission to the implementation of the TRP standards.
• Information about physical activity restrictions.
• The time limit and the description of the restrictions.
• Date of issue of the medical report.
• The date before which the conclusion is valid.
• The position and full name of the person who issued the report.
• Place for the seal of the medical organization, if available.

Sections that have not been filled in are not displayed in printed form.
