Emergency notification of an infectious disease, food poisoning, acute occupational poisoning, or an unusual reaction to vaccination.

How to create a protocol

1. Make an appointment for the patient
2. To form the document, specify the data in the patient's profile:

• Last name, first name;
• Paul;
• Date of birth;
• SNILS;
• The address of the patient's residence/registration.

Create a protocol«Экстренное извещение об инфекционном заболевании, пищевом, остром профессиональном отравлении, необычной реакции на прививку».

You need to fill in all required fields in the document.:

• Type of documented event;
• The start date of the documented event.

In «Сведения о документе»the required fields section:

• Date of initial appeal (detection);
• Type of notification;
• Information about the primary anti-epidemic measures carried out and additional information.

In «Дата и час первичной сигнализации (по телефону и пр.) в СЭС»the required fields section:

• Date/Time;
• The doctor who reported the case.

In «Сведения о неблагоприятной реакции, связанной с иммунизацией»the required fields section:

• Date of drug administration;
• The code of the external cause (adverse reaction during immunization) according to ICD-10;
• Type of MIBP, grouping name, trade name and manufacturer;
• Valid until.

In «Сведения госпитализации пациента»the required fields section:

• Date of hospitalization;
• Name of the medical organization.

• The category of citizenship.

In «Связанные документы»the required fields section:

• Document type;
• Date of issue of the document.

When the conclusion is ready, the doctor and the head of the clinic sign the document to be sent to the RAMD. The document is available for printing in docx format.
