How to create a medical examination report 
1. Make an appointment with a professional therapist.
If necessary, create requests for each scheduled medical check-up visit. The last chronological visit should be to the occupational therapist. In this case, the doctors whom the patient visits as part of the medical examination create a regular examination protocol.
2. To form a medical examination protocol, you must specify the following information in the patient's profile:
- Last name, first name;
- Paul;
- Date of birth;
- SNILS;
- the subject of the Russian Federation in which the patient lives or is registered.
3. The «Заключении по медосмотру»required fields must be filled in:
- date and time of the start of the medical examination;
- date and time of the end of the medical examination;
- structural division of a medical organization;
- type of medical examination;
- name of the type of work;
- the results of examinations by a dermatovenerologist, an otorhinolaryngologist, a dentist, a therapist, indicating the full name of the doctor and the presence / absence of contraindications to work;
- the result of an instrumental study;
- medical contraindications to work have been identified/not identified.
When choosing a type of job that is related to food products, it becomes mandatory to specify the job profile.
If necessary, provide information about past infectious diseases.
Depending on the type of work chosen, it is necessary to provide information about preventive vaccinations and the results of laboratory tests.
4. To provide information about preventive vaccinations and laboratory test results, click «+Добавить»and enter the necessary data.
5. When the conclusion is ready, the doctor and the head of the clinic sign the document. After that«Заключение по медосмотру», he goes to RAMD.
Important! The SAMD 230 is not printed due to the fact that it is used only for the formation of an electronic medical book (ELMC). RAMD sends the document to the subsystem of ELMK FGIS of Rospotrebnadzor, after which the medical book is formed and gets to the patient in Public Services.
