Certificate of absence of medical contraindications for working with state secrets (SAMD 249)

Document Certificate of absence of medical contraindications for work with the use of information constituting a state secret (SAMD 249) is formed in accordance with Order No. 989n dated 26.08.2011.

A certificate is issued by a medical commission based on the results of a citizen's examination by a psychiatrist, a psychiatrist, a narcologist, and a neurologist. According to the order, a certificate is issued if none of the specialists has any medical contraindications from the list.

How to create a document

Make an appointment for the patient.
To form a document, you must specify the following data in the patient's profile:

Last name, first name.
Paul.
Date of birth.
SNILS.
The subject of the Russian Federation in which the patient lives or is registered.
Details of the identity document.

The data of the identity document is required for sending the document to the EGISZ — their availability is checked when signing. You need to fill in all the fields: the series and number, the date of issue, the unit code and the name of the issuing organization. If the data is not filled in, you will not be able to sign the document.

Create a new protocol with the type «Справка об отсутствии медицинских противопоказаний для работы с использованием сведений, составляющих государственную тайну».

Filling out the document

First, fill in the required field «Уровень конфиденциальности»and select a value from the list.
Medical Commission

In this section, provide information about the meeting of the medical commission.:

Date and time of the commission meeting (required for EGISZ).
Date and time of the end of the commission meeting.
The form of medical care (if necessary).
Type of medical care (if necessary).
Conditions of medical care (if necessary).

Then specify the composition of the commission. For each participant, click «+ Добавить»under the heading «Член врачебной комиссии»and select how to specify a doctor:

Select from the directory — select an employee and their position (both fields are required for the Unified State Register of Legal Entities).
Enter manually — fill in the doctor's details if he is not in the directory.

Important! By order No. 989n, the medical commission issues a conclusion on the absence of contraindications. Specify its chairman and members in accordance with the composition of the commission of your medical organization.

Specialist consultations

Be sure to fill out the results of three consultations: a psychiatrist, a psychiatrist, a narcologist, and a neurologist. For each consultation, choose how to specify a doctor — from the directory or manually.

When selecting from the directory, specify:

An employee (a specific employee from your clinic, mandatory for EGISZ).
Position (required for EGISZ).

When entering manually, fill in the details of the doctor and his place of work.:

Last name, First name (required for EGISZ), Patronymic.
Position (required for EGISZ).
The name of the organization (required for EGISZ), the address of the organization.
The subject of the Russian Federation (required for the Unified State Register of Legal Entities), the postal code of the organization.

For each consultation, also specify:

Date and time of the consultation (required for EGISZ).
Conclusion of the consultation.

Comment

If necessary, add a general comment to the document in the appropriate text field. The comment is not printed on the letterhead.

If necessary, add the documents related to the help. Click «+ Добавить документ»and select the document format:

The SAMD is an electronic structured medical document.
Another document.

For each linked document, fill in:

Document type (required for the Unified State Register of Legal Entities when selecting a block) — the list of types depends on the selected format.
Note.
Date of document formation (required for the Unified State Register of Legal Entities when selecting a block).
The series, number, and form of the document.

Printed form

The printed form of the certificate corresponds to Appendix No. 3 to Order No. 989h and is generated in PDF format. The data is inserted into it from the filled-in fields of the document, the data of the medical organization and the patient's profile.

The form displays:

The name, location, postal address, and phone number of the medical organization.
Date of issue of the certificate.
Patient's full name, date of birth, gender, address of the place of residence (stay).
Conclusions on the absence of contraindications from three specialists — a psychiatrist-narcologist, a psychiatrist and a neurologist - with the date of examination, full name and position of each.
An inscription about the conclusion of the medical commission on the absence of medical contraindications.
The chairman of the medical commission and its members, indicating the position and full name.

The chairman of the medical commission shall indicate the author of the document in printed form. The members of the commission are removed from the block«Член врачебной комиссии».

Signing and sending

At the end, add the document's certifier to the field«Заверяет документ».

After filling in all the required fields for the Unified State Register of Legal Entities, sign the document. The document is signed by two signatures: the doctor (the author) and the head of the clinic (or an employee with the right to sign documents on behalf of the head). After being signed by the head, the document is sent to the RAMD.
